ABSTRACT

Objective:To investigate the relationship between Creactive protein(CRP)/albumin ratio (CAR) and lymph node metastasis of gastric cancer. Methods:A total of 96 cases of gastric cancer were included in the study. The clinical pathological stage and lymph node detection in patients with gastric cancer were analyzed, with the preoperative CAR as the dependent variable. Results:1) The pa-tients with preoperative CAR>0.04 has higher transfer rate and metastasis than the patients with preoperative CAR≤0.04, and the dif-ference was significant (P0.04 was significantly higher than that of the patients with preoperative CAR≤0.04, and the difference was significant (P<0.05). 2) The mean CAR of patients with Borrmann typesⅠ,Ⅱ,Ⅲ, andⅣgradually increased;with the progression of the pathological stage of gastric cancer, the mean value of CAR in-creased. 3) During the operation, the total number of lymph node dissections was high and the mean value of CAR before the opera-tion was high. Conclusion:The correlation between preoperative CAR and lymph node metastasis in patients with gastric cancer may reflect the degree of lymph node metastasis to a certain extent.

ABSTRACT

In National Comprehensive Cancer Network guideline, patients with the clinical stages of Ⅰ B-ⅢA esophageal neoplasms should be given radical PF regimen (cisplatin + 5-fluorouracil) concurrent chemoradiotherapy.The majorization PF and radiation dosage are studied by some scholars in China and abroad, and the local control rate and survival rate are improved, adverse reaction is within a controllable range.In recent years, some new drugs such as capecitabine, paclitaxel, docetaxel, pemetrexed and carboplatin also show their advantages in radical concurrent chemoradiotherapy.

ABSTRACT

<p><b>OBJECTIVE</b>To study the safety and feasibility of fast track surgery (FTS) in the promotion of postoperative recovery for gastric cancer patients undergoing gastrectomy.</p><p><b>METHODS</b>From January to December in 2013, 71 gastric cancer patients were prospectively enrolled and randomized into the FTS group and the control group. Patient in the FTS group received FTS management and those in the control group received routine management. The postoperative recovery and stress were compared between the two groups.</p><p><b>RESULTS</b>FTS was associated with shorter time to bowel function return [(67.8±19.7) h vs. (90.0±20.6) h, P<0.01], shorter hospital stay [(13.5±3.0) d vs. (17.8±7.3) d, P=0.01], lower hospital cost [(23.8±3.7) thousand Yuan vs. (27.8±6.1) thousand Yuan, P<0.05], and less stress response (lower pain score, WBC count, C-reactive protein, all P<0.01). The postoperative complications including ileus, infection, anastomotic leakage were similar (all P>0.05).</p><p><b>CONCLUSION</b>Fast track surgery decreases postoperative stress response and promotes recovery.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the better method of digestive tract reconstruction in proximal gastrectomy for early gastroesophageal junction adenocarcinoma.</p><p><b>METHODS</b>A total of 153 cases of early gastroesophageal junction adenocarcinoma who were planned to receive radical proximal gastrectomy from January 2003 to December 2011 were prospectively enrolled and randomly divided into two groups by table of random number according to methods of digestive tract reconstruction, including 3S anastomosis group (80 cases, 3S jejunal interposition) and traditional anastomosis group (73 cases, esophageal remnant gastric posterior wall anastomosis). Postoperative complications, operative time, mortality, nutritional parameters and postoperative quality of life were compared between these two groups.</p><p><b>RESULTS</b>There were no significant differences between two groups in postoperative complications, operative time and mortality (all P>0.05). 3S anastomosis group was better in nutritional parameters than traditional group six months after operation (P<0.05). As compared to traditional group, incidence of reflux esophagitis decreased [20.0%(16/80) vs. 46.6%(34/73), P<0.01] and gastric emptying time prolonged obviously [(160.8±8.1) min vs. (61.1±10.8) min, P<0.01] in 3S anastomosis group 18 months after operation. Postoperative QLQ-C30 rating scale revealed quality of life was significantly higher in 3S anastomosis group as compared to traditional group.</p><p><b>CONCLUSION</b>Jejunal interposition is a better method of digestive tract reconstruction in proximal gastrectomy for early gastroesophageal junction carcinoma.</p>

ABSTRACT

Objective To investigate the relationship between expression of DAS-1 in gastric cardia intestinal metaplasia(CIM)and gastric cardia adenocarcinoma (GCA). Methods The cancerous tissues and CIM tissues (2 cm apart from caneer) obtained from 65 patients with GCA were examined for the expression of DAS-1 protein using immunohistoehemistry. The CIM tissues (<2 cm below Z line) obtained from 15 outpatients and inflammatory mucosa from 25 outpatients were also examined for expression of DAS-1 protein. Results The type Ⅲ IM was accounted for 55.4% (36/65) in GCA patients, which was significantly higher than that in outpatients [13.3% (2/15), P<0.01]. The positive rate of DAS-1 expression in cancerous tissues [78.5 % (51/65)] was also significantly higher than that in CIM tissues [38.8 %(30/80), P<0.01]. The expression of DAS-1 protein in IM tissues was gradually increased from type Ⅰ (0%) to type Ⅲ (71.1%) with positive correlation (P<0.01). Conclusions The type Ⅲ IM with over-expression of DAS-1 is closely related to GCA, which might be one of important precancerous lesions for GCA.

ABSTRACT

Objective To study the effects of Genistein - magnetic - nanoparticles at different concentrations on the growth and ex-pression of focal adhesion kinase (FAK) in human hepatocellular carcinoma cell line HcpG2. Methods Activities of HepG2 cells treated by Genistein - magnetic - nanoparticles were examined by MTT assay. The expression of FAK mRNA and protein was respectively detected by immunohistochemistry staining and reversed transcriptase polymerase chain reaction (RT - PCR) . Results Genistein - magnetic -nanoparticles at a concentration of 10 -80mg/L inhibited the proliferation of HepG2 cells, with obvious concentration - dapendent and time - dependent effect relationships (P < 0.05). After treatment with various concentrations of Genistein - magnetic - nanoparticles for 48h, the relative level of FAKmRNA of Genistein - magnetic - nanoparticles and control groups was 1.242 ± 0.031,1.213 ± 0.443,0.834 ± 0.044,0.652 ± 0.039,0.446 ± 0.041, and 1.443 ± 0.053 (F = 21.97 ,P < 0.05), respectively. The expression of FAK protein in the cells was decreased, which showed an obvious a concentration - effect relationship. Conclusion Genistein - magnetic - nanoparticles can reduce the mRNA and protein expressions of FAK and inhibit the proliferation of HepG2 cells.

ABSTRACT

Objective To investigate the sensitivity of tumor cells to photodynamic therapy(PDT), and to select the optimal photo-dose and photo density for PDT to the cultured cells. Methods Cells of SHEE and SHEEC cell lines were divided into 21 groups randomly,after 24 h inoculation, the cells accreted on paries of culture capsule completely, we replaced M199 complete culture solution with 30 μg/rnl Photofrin-Ⅱ solution 100 μl and then replacd with M199 complete culture solution without Photofiin-Ⅱ after 150 min of incubation in Photofrin-Ⅱ. Within 15 min,we dealed the cells with PDT using Photofrin-Diomed 630 under three different power densities:25 raW/cm~2,50 mW/cm~2 and 100 mW/cm~2 for 10 s,20 9,30 s,50 s, 100 s, 150 s and 200 s respectively,and then continue for 24 h culture. We examined the inhibiting effect on cell line SHEE and SHEEC under PDT by the method of CCK-8. Results There was no significant difference in the inhibition ratio with same power density of PDT between cell line SHEE and SHEEC under the concentration of 30 μg/ml Photofrin-Ⅱ. However, the inhibition ratio increased with the raising of photo-dose of PDT,but there was a platform stage after the 2.5~30 J/cm~ 2 photo-dose. Conclu-sion The difference for photodynamic sensitivity between human immortalization esophageal epithelial cell line SHEE and the malignant transformation cell line SHEEC is not significant. The targeting of PDT to malignant tumor cell may not be involved in the photosensitivity for tumor cell.

ABSTRACT

BACKGROUND:Diabetic acromelic gangrene(diabetic foot)has become one of the important causes for the disability and death in diabetes mellitus.OBJECTIVE:To establish model of diabetic foot in rat,and observe the interventional effect of Chinese compound on diabetic foot.DESIGN:A comparative observational experiment.SETTING:The First Affiliated Hospital of Clinical Medical College,Henan University of Science and Technology.MATERIALS:Fifty male Wistar rats of 8 weeks old,(200.0±16.3)g,were raised with common feed in separate cage at the room temperature of 18-22℃.and they were free to access of feed and water.Ten rats were randomly selected as the blank control group,and the other 40 were used for model establishment.METHODS:The experiments were carried out in the Animal Room of Henan University of Science and Technology from October 2001 to April 2002.①Grouping:The 40 rats were fasted for 6 hours,and then treated with intraperitoneal injection of streptozotocin(55.0 mg/kg),while the 10 rats in the blank control group were injected with isovolume sodium citrate buffer solution.20 models were successfully established,and they were randomly divided into model group(n=10)and treatment group(n=10).Rats in the treatment group were treated for 3 weeks with intragastric perfusion of Chinese compound(60 g/kg)at 9:00 every day after model establishment,and those in the model group were given intragastric perfusion of isovolume saline.At the end of the third week,the rats were all killed under anesthesia after fasted for 12 hours,blood samples were collected to determine the levels of fasting blood glucose,blood lipids and insulin.The daily amount of drinking water was recorded in each group during the experiment.②Scoring standards for acromelic gangrenes:The limbs rats with diabetic foot were scored by three grades,including dark skin,mild open focus on skin,and focus had invaded deep muscular tissue.The total score of each rat was calculated.The beta-cell function index (HBCI)was also evaluated.MAIN OUTCOME MEASURES:The changes of the amount of drinking water,body mass and levels of triglyceride,cholesterol,fasting blood glucose,blood lipids and insulin were observed before and after treatment.RESULTS:Totally 50 Wistar rats were used.20 of them were excluded due to unsuccessful model establishment,and the other 30 rats were involved in the final analysis of results.①The amount of drinking water was obviously higher in the model group and treatment group than in the blank control group during the experiment(P<0.01).As the treatmentlasted,the amount of drinking water was obviously decreased in the treatment group,but gradually increased in the model group.②After treatment,the body mass was obviously lower than that before treatment in the model group(P<0.01).but had a descending trend without obvious difference as compared with that before treatment in the treatment group(P>0.05).③Obvious acromelic gangrenes were obvious in both groups(P<0.01).The body mass in the treatment group was obviously higher than that in the model group(P<0.01),and the conditions of acromelic gangrene were obviously better than those in the model group(P<0.01).④Before treatment,the levels of fasting blood glucose in the treatment group and model group were obviously higher than that in the blank control group(P<0.01),while the levels of insulin and HBCl were obviously lower than those in the blank control group(P<0.01).After treatment,the levels of fasting blood glucose in the treatment group and model group were obviously higher than that in the blank control group(P<0.01),and it was obviously lower in the treatment group than in the model group(P<0.01),it was close to the normal value in the treatment group.⑤The levels of triglyceride and cholesterol before treatment were obviously higher in the treatment group and model group than in the blank control group(P<0.01).After treatment,the levels of triglyceride and cholesterol in the treatment group were obviously decreased(P<0.01), which were not obviously different from those in the blank control group (P>0.05), while those in the model group were increased continuously,and obviously higher than those in the blank control group(P<0.01).CONCLUSION:Increasing the serum level of insulin and decreasing the levels of blood glucose and blood lipids can prevent and treat the occurrence and development of diabetic foot to some extent.This model of diabetic foot is sensitive to drug,and can be used to investigate the pathogenesis of diabetic foot and evaluate the effect of drug therapy.
